What Every Shape-Up Member Should Know — Updated Edition
We’ve been talking about creatine at Shape-Up for a long time — well before the current wave of research made it fashionable to talk about. For years, when a member over 50 asked what one supplement I’d recommend they discuss with their doctor, my answer was almost always the same: creatine. Not because it was trendy. Because I’d watched what it did for members who took it consistently alongside their training, and I’d seen the underlying science steadily catch up to what our floor was already telling us.
Today the science has arrived in a big way. The research on creatine — for muscle, for brain, for healthy aging — is some of the most exciting work in the field. And with more of our members over 50 now taking it, the questions have shifted from “should I?” to “am I taking it right?” This article is my attempt to answer that in one place, drawing on both the research and everything I’ve learned watching thousands of members work through it.
Fair warning: this is going to be a fuller piece than usual. Creatine deserves it.
Why This Matters More After 50
Let’s start with the muscle side, because that’s where the story begins.
Beginning around age 30, we lose roughly one percent of our muscle mass per year — about a third of a pound annually — if we do nothing to counter it. That doesn’t sound like much until you do the math over three or four decades. What makes it worse is that we don’t lose muscle evenly. We preferentially lose our fast-twitch (Type II) fibers — the fibers that generate power, catch us when we stumble, get us out of a chair, and let us climb a flight of stairs without pausing. By age 80, most people have lost half of their skeletal muscle, with the fast-twitch fibers taking the brunt of the damage. This process — sarcopenia — is one of the primary drivers of frailty, falls, and loss of independence in later life.
Here’s the good news, and it’s the news I’ve been sharing on the Shape-Up floor for decades: this is reversible. Resistance training, done consistently and with progressive overload, doesn’t just slow this loss — it can reverse it. The fast-twitch fibers grow back. Strength returns. Function returns. This is not opinion; this is settled science.
That’s the foundation. Creatine is what you add on top of that foundation to get more out of it.
The One Idea That Unlocks Everything
If you understand nothing else about creatine, understand this: creatine is a system you fill, not something you feel.
Creatine is not caffeine. You don’t take a dose and feel it kick in an hour later. What you’re actually doing is gradually filling a reservoir inside your muscle tissue — a finite pool your body uses to regenerate ATP, the currency of cellular energy, during every hard effort you make. Most people are walking around with that reservoir only partially full. When you supplement consistently, you fill it to the top and keep it there. Then, on every set of squats, every set of stairs, every time you lift a grandchild, your muscles have more energy to draw on.
That’s the muscle side. The brain side works on a similar principle but with an important twist we’ll get to below: your brain needs more creatine to fill its reservoir than your muscles do.
How to Actually Take It
Here’s where the research has evolved meaningfully in the last two years, and where I want to be careful to give you the updated picture. There are now two clear tiers to consider, depending on what you’re after.
Tier One — For Muscle, Strength, and Body Composition: 3–5 Grams per Day
This is the classic recommendation, and for the muscle side of the story it remains right. Three to five grams per day, taken consistently, saturates your muscle creatine stores within three to four weeks. This dose supports strength, lean mass, recovery, and body composition. For members whose main goal is what happens on the training floor, this is enough.
Tier Two — For Cognitive and Brain-Health Benefits: 10–20 Grams per Day
This is the newer picture, and it deserves your attention. Here’s why the dose is so much higher: only about five percent of the creatine in your body ends up in your brain, and the transporter that carries creatine across the blood-brain barrier operates near saturation. That means the dose needed to meaningfully elevate brain creatine is substantially higher than the dose needed to fill muscle.
The recent research on brain creatine and cognition has clustered in the 16–20 gram-per-day range for studies that produced measurable changes. A practical middle ground many practitioners are now recommending is 10 to 15 grams per day, which balances brain uptake with cost and GI tolerance. For members over 60 who are interested in the cognitive benefits — and given what the research is starting to show, I think most of you should be — the higher end of this range is where I’d point you.
If you’re going to take a higher dose, split it across the day (say, 5 grams three times a day for a 15-gram target) rather than taking it all at once. This substantially reduces the odds of GI discomfort and improves absorption.
Timing
Take it when you’ll remember. There’s a small edge in the research for taking creatine post-workout with a meal, but the effect is minor. What matters far more is that you take it every day. I take mine with my post-workout protein shake, which doubles as a reminder I never miss. Attach it to something you already do.
Loading
Not necessary for most. You can front-load with 20 grams per day for a week to saturate muscle stores faster, but unless you have a near-term reason to be at peak levels next week, there’s no benefit over simply starting your daily dose and letting the tank fill naturally over three to four weeks. Note that if you’re using a Tier Two brain-health dose (10–20 g/day), you are effectively “loading” every day anyway.
Cycling
Don’t. I get this question every single month: “Should I take a break?” No. There is no scientific basis for cycling creatine. Your body doesn’t down-regulate anything in a way that makes breaks necessary or helpful. If you stop, your stores gradually return to baseline over four to six weeks, and you spend that time running below your ceiling for no reason. I do believe in strategic rest from certain nutrients — but creatine and protein are not on that list. These are foundational. You take them consistently, the way you train consistently.
About That Creatinine Number on Your Bloodwork
This is one of the most common questions I get, and it’s important enough that I want you to have the answer before it worries you.
When you have bloodwork done, one of the numbers on the panel is serum creatinine — a routine marker of kidney function. If you’re supplementing with creatine and your creatinine number comes back slightly elevated, the natural reaction is alarm. I’ve had members call me genuinely worried they were damaging their kidneys.
Here’s what’s actually happening. Creatinine is a normal metabolic byproduct of creatine. When your muscle creatine stores are saturated, you produce and excrete slightly more creatinine. This shows up on the blood panel as a modestly elevated number. It is not a sign of kidney dysfunction — it is a sign the creatine is doing exactly what it’s supposed to do.
In healthy people with normal kidney function, creatine at standard doses has been studied for decades and has not been shown to cause kidney damage. If you have pre-existing kidney disease, that’s a real conversation to have with your doctor. For everyone else, the smart move is simple: tell your physician you’re supplementing with creatine before the blood draw, so they can interpret the result correctly.
The Brain Health Revolution
Now we get to what may be the most exciting frontier in creatine research: what it’s doing for the brain.
Your brain is one of the most energy-hungry organs in your body. Like your muscles, it uses creatine to help regenerate ATP. And like your muscles, brain creatine content appears to decline with age — which some researchers believe may be linked to reduced brain activity and increased vulnerability to neurodegenerative disease.
The research on creatine and cognition in older adults is compelling and getting more so:
- A pilot study at the University of Kansas Medical Center (Smith et al., 2025) enrolled 20 participants with Alzheimer’s disease, average age 73, and gave them 20 grams per day for eight weeks. The result: an 11% increase in total brain creatine, along with statistically significant improvements in global cognition, fluid cognition composite, working memory (list sorting), oral reading, and executive function (Flanker test).
- Earlier work by Dechent and colleagues found that 20 grams per day for four weeks increased brain creatine by 8.7% in healthy adults — the first solid demonstration that oral creatine could measurably move brain creatine levels.
- Meta-analyses of memory and attention studies show significantly greater cognitive benefits in older adults (66–76) than in younger individuals (11–31). The older you are, the more your brain appears to respond.
- A 2025 review in the Journal of Psychiatry and Brain Science, titled “Creatine Supplementation: More Is Likely Better for Brain Bioenergetics, Health and Function,” summarizes where the field is going: for meaningful brain effects, the standard 3–5 gram muscle-saturation dose is likely insufficient, and researchers are moving toward the 10–20 gram range.
- A 2026 systematic review in Nutrition Reviews on creatine and cognition in aging concluded that larger doses may be needed to cross the blood-brain barrier and elevate brain creatine, and suggested a relative dosing strategy tailored to body weight may be more appropriate for older adults than a one-size-fits-all approach.
- Emerging evidence suggests those with lower baseline brain creatine — often older adults and vegetarians — may respond most strongly to supplementation.
I want to be careful not to oversell this. The research is still developing, and creatine is not a treatment for Alzheimer’s disease. But the mechanism makes sense, the early results are pointing in a real direction, and the risk for a healthy adult is essentially zero. This is one of the reasons I’m now recommending creatine to virtually every member over 50 who asks — not just for the muscle benefits, but for what it may be doing upstairs.
One important caveat: a 2023 dose-response study in healthy young adults (Moriarty et al.) compared 10 g/day, 20 g/day, and placebo for six weeks and found no significant cognitive effect at either dose. The most likely explanation is that healthy young adults already have brain creatine near ceiling. The people most likely to respond to higher doses are older adults, vegetarians, and those under cognitive stress — which describes a very large portion of our membership.
Creatine and Weight Loss (Including GLP-1 Medications)
A word to members who are actively working on body composition, whether through traditional weight loss efforts or with the help of GLP-1 medications like Ozempic or Mounjaro. Creatine does not cause weight loss. What it may do is help preserve lean muscle mass during periods of rapid weight reduction — and that’s a big deal.
One of the well-documented concerns with rapid weight loss, especially on GLP-1s, is that a meaningful portion of the weight lost can be muscle rather than fat. Losing muscle in your 50s, 60s, and 70s is exactly the wrong direction. Combined with adequate protein and resistance training, creatine helps tilt the balance toward preserving the muscle you want to keep while you shed the fat you want to lose. The Supporting Cast: Protein and Vitamin D
Creatine does not work in a vacuum. In fact, it works far better as part of a system. Two other pieces are non-negotiable for me:
Protein
If you are under-eating protein, no amount of creatine will build or preserve muscle for you. For members over 50, I consistently recommend aiming for roughly one gram of protein per pound of goal body weight, spread across three or four meals a day. This is higher than the government RDA — deliberately. The RDA was designed to prevent deficiency, not to preserve muscle as we age. Higher protein needs are one of the defining features of aging well.
Vitamin D
In my recent piece on Bob Delmonteque — the photograph of my father, Bob, and me at nineteen at the Jack LaLanne Spa opening — I touched on why vitamin D matters so much at our stage of life. Deficiency is rampant in this country, particularly in older adults. Low vitamin D is associated with reduced muscle function, poorer recovery, more falls, and worse cognitive outcomes. If you haven’t had your 25-hydroxy vitamin D checked recently, ask your doctor. If you’re low — and most people are — fix it.
Creatine + protein + vitamin D + resistance training. That is the package. Any one of them alone is doing less than the four of them together. This is what I mean when I talk about Long-Term Fitness. It’s not one thing. It’s the right things, consistently, over time.
What I See on the Floor
After decades of watching members work with creatine, here’s what actually happens for the ones who do it right:
They don’t feel dramatic overnight changes. What they notice, four to six weeks in, is that their training feels a little better. They get one more rep. Their recovery between sets is a bit sharper. Over months, their body composition trends in the right direction more decisively than it did before. They feel more like themselves in the gym.
The members who struggle with creatine are almost always struggling with consistency, not with creatine itself. They start and stop. They cycle for reasons that have no basis. They convince themselves it “isn’t working” three weeks in — before their tank is even full. The ones who quietly take their daily dose, every day, for a year — those are the ones who become believers.

The Bottom Line
At Shape-Up, we have always believed in taking a proactive approach to aging. We have believed that growing older does not mean growing weaker, less capable, or less sharp. The emerging science on creatine — muscle, brain, longevity — is one more piece of evidence that this belief is correct. But belief without action doesn’t build anything.
The action is simple. Pick your tier — 3 to 5 grams a day for muscle and body composition, 10 to 15 grams a day if you want the cognitive benefits too. Take it every day. Get your protein in. Get your vitamin D checked. Show up for your training. And let biology do what biology does.
